What is it?
The Long Term Conditions self-management Course is for people living with a long term physical health condition who would like to help themselves better manage their long term condition and wellbeing. The course is 9 sessions over 9 weeks, each session is 2 hours long from 14:00-16:00. To fully benefit everyone in the group, we ask for commitment to all sessions. There are usually between 7-14 people who will attend.
Who is this suitable for?
This course is suitable for anyone who lives with a long term condition who is interested in taking a more active approach in the self-management of their condition. We know that living with a long term condition can be challenging and stressful at times. This may be because the health condition itself can be demanding to manage and/or the condition may be having a significant impact upon many different aspects of life including routine activities, mood, energy, sleep and relationships.
How will this help me?
We hope this course will make a real practical difference in your day-to-day management of your condition and your psychological well-being. This group is based uses the principles of Acceptance and Commitment Therapy (ACT). This is a way of learning to live a full and meaningful life whilst managing the emotional difficulties of having a long term health condition, such as worry, frustration, fear and low mood. On this course you can expect to:
- Learn what it means to live with and manage your long term condition, for example, learn how to take a problem solving approach to overcome difficulties or manage your energy levels through pacing activities.
- Learn helpful ways to manage your psychological well-being.
- Meet and share learning with other people living with a long term condition.
